- 3-Year Uncertified Transcript: $14.98 ($8.00 transcation fee +$6.25 tax collector service fee + $0.73 processing fee)
- Complete Uncertified Transcript: $17.04 ($10.00 transaction fee + $6.25 tax collector service fee + $0.79 processing fee)
- A complete transcript includes all Florida driving history, including any open or closed citations/sanctions that have not met their retention period.
- Your card will only be charged if your transaction is successful.

Important Restrictions
- This form is not for certified transcripts. Visit MyDMV Portal for certified copies.
- Transcripts are only available to the record owner. Parents seeking their child’s record must use MyDMV Portal.
- Our office cannot interpret transcript content. For help, visit flhsmv.gov/ddl/abstract_questions.html.
- No Refunds: Once submitted, your transaction cannot be refunded, voided, or reversed.
